To participate in the PolyTransfer Advantage Summer Academy, you must:
1. Submit an Application You must complete and submit this form.
2. Be Enrolled at a California Community College You must be enrolled at a California Community College (CCC). You will be asked to provide your CCC student ID number and CCC email address on the application.
3. Age Requirement You should be at least 18 years of age at the start of the program.
Thank you for your interest in the PolyTransfer Advantage Summer Academy. Based on the birthdate provided in your application, it appears you will be under 18 years of age at the start of the program. At this time, your application will require additional review by the PolyTransfer Advantage programming committee to determine whether we can accommodate your participation in this year’s program. A member of our team will follow up with you soon regarding the status of your application.
As part of this program, we are offering the Arise Summer Academy for Asian, Pacific Islander and Desi American students.
If you are under 18 years of age, this must be signed by your parent/legal guardian.
